Documents necessary for every Foreign student

Here is a list of all documents to be sent to the University Of Sacred Heart before coming to San Juan. Please be sure that all documents are translated into english before sending them.

  1. Birth certificate
  2. Copy of passport
  3. Affidavit of economic support
  4. Admission application form from Sacred Heart University
  5. Curriculum vitae
  6. Official transcript from your university
  7. Letter of recommendation from your university
  8. Class schedule for the term at U.S.C.
  9. Residence application form, (for housing on campus) and Residence Hall Service Agreement ( application fee $125)
  10. Repatriation Insurance Policy as required by the State Department in order to issue a permit IAP-66 (U.S. $110.00). The student only has to send the fee and the university will will included him in the policy.

These documents should arrive here well before the students arrive, so that we can prepare the application for a visa. The dead-lines for receiving the documents are:

Spring Semester 2002 : September 15, 2001

Fall Semester 2001 : April 30, 2001
